Independent Supported Living
Independent Supported Living
All services provided by HILA to its service users are provided under a person-centred approach.
The approach provides a holistic and people-oriented approach to planning, within which decisions about the help people need and how to best deliver that help are led by the person himself/herself. Whereby, rather than focusing on what people cannot do, the emphasis is on the inherent worth and dignity of each person as an individual, their gifts, and capacity to contribute and be self-sufficient.
HILA’s main aim is to empower and support the service user to achieve his/her full potential so that, together, the perfect recipe can be found to ensure that the service user achieves self-respect and direction to be able to live a dignified and fulfilling life.
The way HILA achieves this is by involving service users, families, and the multi-disciplinary team in compiling an Assessment of Needs which then create a personal support plan which is reviewed on a 6-monthly or yearly basis according to the needs of the service users. HILA strongly believes in the individuality of each, and every service user and this involvement ensures that the person-centred approach is used, highlighting their unique strengths and needs.
HILA provides supervised supported care for twenty-four hours per day, seven days per week, and 52 weeks per year including public holidays.
The supervised supported services include:
• The provision of accommodation on a full board basis (breakfast, lunch, afternoon tea and dinner). Special diets are also provided to meet the needs of all service users.
• The provision of necessary equipment/aides, medical or otherwise to cater for the service users’ needs and to encourage independence.
• A detailed tailor-made personal support plan including the service users’ needs and wishes; the support needed both in relation to equipment and human resources; food preferences and any dietary requirements; social, psychological, cultural, and spiritual preferences and the administration of medication. This plan is drafted with the service user and their families where possible together with internal and external members of the multi-disciplinary team.
• Provision of care in all aspects of daily living required by the service users, including nursing care and the assistance in collection of pharmaceuticals and medical equipment/consumables for those clients entitled to free drugs, from POYC where patients will be registered.
